Organizations may still need separate tooling for HL7 v2 routing, complex transformations, and operational interface monitoring. This can add architectural complexity when supporting broad interoperability requirements. It is better positioned as a data store and normalization layer than as an end-to-end integration hub.
Requires data engineering expertise
Implementations typically require skilled data engineering to map source systems, manage data quality, and design downstream analytics models. FHIR normalization does not automatically resolve semantic differences across source systems, coding practices, or incomplete data. Teams often need governance processes for terminology, deduplication, and longitudinal patient matching. Total cost and time-to-value depend heavily on internal capabilities and the complexity of source data.
