How pricing is determined (official): Monthly Managed Service Fee is calculated by multiplying the Customer’s Form A Line 33 Amount for the current Service Year by the Monthly Managed Service Fee Rate on the Monthly Service Fee Table in Exhibit B of the Order Form, then dividing by twelve (12).
